Inflammatory Bowel Disease

  • Ulcerative colitis — continuous inflammation from rectum proximally, mucosa-only, bloody diarrhoea, associated with PSC; smoking is protective. Toxic megacolon and colorectal cancer are key complications.
  • Crohn’s disease — skip lesions anywhere from mouth to anus, transmural inflammation, granulomas, fistulae and strictures; smoking worsens it. Terminal ileal disease causes B12 and bile-salt malabsorption.
  • Extra-intestinal features — erythema nodosum, pyoderma gangrenosum, anterior uveitis, enteropathic arthritis. Some track disease activity, some (PSC, ankylosing spondylitis) do not.

Malabsorption & Coeliac Disease

  • Coeliac disease — anti-tissue transglutaminase (anti-tTG) IgA antibodies (check total IgA to exclude deficiency), duodenal biopsy showing villous atrophy and crypt hyperplasia, HLA-DQ2/DQ8, association with dermatitis herpetiformis. Iron, folate and B12 deficiency are common.
  • Whipple’s disease — Tropheryma whipplei, middle-aged man, arthralgia, weight loss, diarrhoea, PAS-positive macrophages on biopsy.
  • Bacterial overgrowth — raised folate, low B12, positive hydrogen breath test, responds to antibiotics.

Upper GI, Drugs & Miscellaneous

  • Helicobacter pylori — peptic ulcer disease, MALT lymphoma; eradication with a PPI plus two antibiotics. Stop the PPI for 2 weeks (and antibiotics for 4 weeks) before urea breath/stool antigen testing.
  • Carcinoid syndrome — flushing, diarrhoea, wheeze; raised urinary 5-HIAA, right-sided heart valve lesions when hepatic metastases are present.
  • Drug effects — NSAIDs cause peptic ulceration; metformin and orlistat cause diarrhoea; opioids and ondansetron cause constipation. Dietary sodium restriction is first-line for cirrhotic ascites, with spironolactone the first-line diuretic.

How to Approach Gastroenterology & Hepatology Questions

  1. Read the numbers first. Before the clinical story, classify any LFTs as hepatocellular or cholestatic and note albumin and INR for synthetic function. The pattern usually halves your differential before you reach the diagnosis.
  2. Map the demographics. Middle-aged woman with itch and AMA is PBC; young man with UC and beaded ducts is PSC; bronze-skinned diabetic with arthropathy is haemochromatosis. The examiners signpost with age and sex deliberately.
